About our office
The DermacenterMD office is available Monday
through Friday from 8:00am to 5:00pm for your skin care needs.
Dr. Roger Moore performs surgeries in the mornings and makes
time available for cosmetics on a daily basis. You may contact
our office at 574-522-0265.
The office of DermacenterMD provides a unique
and warm atmosphere for your cosmetic and medical visits.
You will find a client lounge complete with a soothing fountain
and couch to provide a relaxed setting during your visit.
The comfortable setting reduces anxiety for our clients at
the visit and during procedures. In addition we utilize state
of the art equipment including the latest cosmetic and laser
technology. Our Mohs laboratory maintains a high level CLIA
certification.
Financial Information
Your treatment plan will include a breakdown
of all applicable fees, and we will inform you of all costs
before treatment is administered. Charges are payable at the
time of treatment. You may also pay upon receipt of EOB from
insurance.
We accept Discover, Visa and MasterCard and
routinely offer special offers for our cosmetic procedures.
If special arrangements are needed, please talk
to our office manager prior to receiving service.
Insurance
We will submit all claims for “in network”
insurers. We will fully attempt to help you receive full insurance
benefits; however, you are personally responsible for your
account, and we encourage you to contact us if your policy
has not paid within 30 days.
